A study to assess the effectiveness of structured educational programme on knowledge of nurses working in cardiac units regarding anxiety and depression among patients having myocardial infarction

Abstract


Context: In India a significant anxiety (48.5%) and depressive symptoms (25.2%) are reported
among the myocardial infarction patients. The high level of co-morbidity between myocardial
infarction and depression also has clear implications for patients' quality of life. Depression has an
important impact on cardiac outcome. Health workers in particular, nurses, if provided with proper
training, may be effective in treatment of psychological responses to an acute myocardial infarction.
Anxiety and depression may go unrecognized in patients hospitalized for acute myocardial infarction
unless specifically assessed as part of usual care.

Aim: Assess the effectiveness of structured educational programme on the knowledge of nurses
working in cardiac units regarding anxiety and depression among patients having myocardial
infarction.
Settings and Design: This pre experimental (pre-test and post-test) research study was undertaken
among 40 nurses working in Cardiac Units, Tertiary Care Hospital, sampled conveniently.
Methods and Material: Structured educational programme was developed by the researcher. Data
was collected using self-developed prevalidated and pretested subject data sheet, semi-structured
knowledge questionnaire and opinnionaire on acceptability of structured educational programme.
Baseline knowledge was assessed on day 1(Pre-test). Then nurses were instructed to do self-reading
about the topic from any other source (intervention part 1) and post-test 1 was done on Day 14
followed by the structured educational programme (intervention part 2), then immediate post-test 2
was done on day 14 followed by post-test 3 on day 35.
Result: There was significant gain in the knowledge score immediately after as well as 3 weeks after
structured educational programme, both from the baseline(pre-test 1) and after self-reading (Post-test
1). Structured educational programme was found effective in improving the knowledge of the nurses
(p<0.05) as compared to self-reading. There was no significant association of the structured
educational programme with the selected variables, however there was a weak negative correlation
between the working experience of the nurses and the knowledge score. The structured educational
programme was found highly acceptable among the nurses.
Statistical analysis used:
Data was analysed using STATA version 11. Descriptive statistics i.e., frequency, percentage, mean,
median, standard deviation, and inferential statistics i.e., repeated ANOVA, Mc Nemar, chi square
test were used to study the association of the knowledge of nurses with selected variables.
Conclusion: There was a gap in nurses’ knowledge especially the nursing responsibilities related to
anxiety and depression among patients having myocardial infarction. The structured educational
programme was found effective in improving the knowledge of the nurses.
