Conceptual framework: Modified Ludwig von Bertanlanffy general system’s model. Methodology: A quantitative approach, pre-experimental one-group pretest–posttest design was used in this study. Sixty subjects were selected at Government Higher Secondary School, Paravai, Madurai, using purposive sampling. After obtaining the informed consent from their parents, pretest was conducted on the first day using a structured knowledge questionnaire. From the second day, snake and ladder game on environmental sanitation was given to the subjects for 3 days. Posttest was conducted on the eighth day after the third day of intervention. Results: The calculated t-value (22.709) was much higher than the table value at 0.001 (preset level of significance was 0.05). There was a significant association between the level of knowledge and selected socio-demographic variables such as occupational status of father and mother. Conclusion: The study concluded that snake and ladder game on environmental sanitation was effective on increasing the knowledge among middle school children.
