Abstract

This study aimed to design and validate, by a panel of experts, an algorithm to support the clinical practice of the nurse, that promotes incident prevention in community-dwelling elders. We carried out two studies: an integrative review of literature, for the design of the algorithm, which was validated by a panel of 8 experts, using the nominal technique. Ethical procedures have been taken into account. The analysis and synthesis of the 26 studies constituting the bibliographic sample allowed us to identify effective interventions in the prevention of falls along hospital-community transition. The final algorithm was validated by the panel of experts unanimously. Continuity of care is vital to prevent falls or recurrence in hospitalized elderly people who are at high risk of falling or who fell during the hospitalization. Research on falls after discharge is needed.
